Rhode Island's medical cannabis card is administered by the Cannabis Control Commission (CCC) through the Cannabis Licensing Portal. The process is straightforward, but the in-person physician requirement is a key step that cannot be skipped.

Step 1: See a Physician In Person

Visit a Rhode Island-licensed MD or DO for an in-person evaluation. Telemedicine is not permitted for initial certifications — you must physically attend the appointment. Bring documentation of your qualifying condition (medical records, prior diagnoses, imaging results, etc.).

If the physician determines you have a qualifying condition and that cannabis therapy is appropriate, they will issue a written certification that you will submit with your application.

Physician consultation fees typically range from $75 to $200, depending on the provider. This is the only out-of-pocket cost, since the state has eliminated all card fees.

Step 2: Apply Online

Register at the Cannabis Licensing Portal (cannabislicensing.ri.gov). Create an account and complete the patient application form.

Step 3: Upload Required Documents

  • Proof of Rhode Island residency — government-issued ID showing a Rhode Island address
  • Physician certification — the written certification from your in-person evaluation

Step 4: Submit (No Fee Required)

Since December 2022, Rhode Island has eliminated all patient registration fees. There is no application fee, no annual renewal fee, and no processing fee. The only charge is a $10 replacement fee if you lose your card.

Renewal

Rhode Island medical cards must be renewed annually. The renewal process mirrors the initial application: obtain a new physician certification (in-person visit required), submit the renewal through the Cannabis Licensing Portal, and wait for your new card. Renewal fees are $0.

The CCC sends renewal reminders, but it is your responsibility to renew before your card expires. If your card lapses, you will need to reapply as a new patient.

Important Notes

  • Rhode Island residency required: You must have a Rhode Island address on a government-issued ID. Out-of-state residents cannot apply for a Rhode Island card (but Rhode Island does accept out-of-state medical cards for purchasing purposes).
